In this case the insured loss shall be defined as death or injury of the insured as a result of an accident that occurred during the period of validity of the insurance contract and is confirmed by documents issued by competent authorities (medical institutions, law-enforcement bodies).
The accident shall be understood as:
trauma;
drowning;
burns, traumas caused by lightning or electric current;
frostbite, supercooling;
accidental acute poisoning by chemical agents (industrial or domestic), medications;
animal, venomous insect, snake bites.
Death or trauma of the insured, which are related to the below stated circumstances, SHALL NOT be regarded as accidents:
participation of the insured in strikes, rebellions, domestic disturbances, sabotages, acts of terrorism;
civil wars or war with another states;
consequences of radiation exposure and other emergency and unavoidable events (natural disasters);
commission of an intended crime;
driving self-propelled vehicles under the influence of liquor, drugs or toxicants, or transfer of control over the motor vehicle to another person under such influence;
willful infliction to himself/herself of bodily injuries.
Diseases (except for tetanus, hydrophobia, tick-borne meningoencephalitis) as well as death from disease or age SHALL NOT be regarded as the insured loss.
WHAT SHOULD BE DONE?
In case of the onset of the insured loss one should:
apply to a medical institution for first medical aid and treatment or call in an ambulance and motor licensing and inspection department personnel in case of a traffic accident;
immediately notify a branch or a regional office of the "Oranta" Incorporated of the event that has occurred;
after completion of treatment collect documents necessary for investigation of the insurance case and submit them to the "Oranta" Incorporated's branch.
IF YOU ARE ABROAD
In case of the onset of the insured loss or a sudden disease you should get in touch with the Assistance Department Elvia Poland (and NOT with agents and NOT with other subdivisions of Elvia) by telephone: +48 22 522 25 44.
The cost of emergency medical aid services abroad may be reimbursed only in case of timely contact with Elvia Poland and coordination of treatment.
If for any reason you are not able to get in touch with Elvia, you should apply to the nearest medical institution (the public one is advisable) and produce an insurance policy to the doctor. Ask the doctor to get in touch with the Elvia representation.
The cost of calls and faxes to Elvia Poland may be reimbursed in case corresponding confirmations (telephone bills) are provided.
DOCUMENTS NECESSARY FOR INVESTIGATION OF THE INSURANCE CASE:
issued by medical institutions that provided first medical aid and gave treatment — Certificate (form ¹ 195), an extract from the hospital patient's card, X-ray images, Certificate of temporary loss of working capacity;
in case of the insured's death (for heirs and beneficiaries) — Death Certificate, Certificate of the Right to the Estate issued by a notary's office;
in case of disability — Certificate of Disability issued by specialized institutions;
in case of an industrial injury — Accident Report Form;
a certificate issued by internal affairs bodies (if necessary);
a written claim of the set form;
other documents evidencing the fact and circumstances of the onset of the insured loss, the amount of expenses etc. — upon request of the insured.
